Weekly Accountability Reflection

The purpose of this form is a tool to bring you awareness and accountability. We’ll use it to help us have productive sessions and track your progress. Reflecting allows you to celebrate your success and better pinpoint how to improve.

The prompts are optional to fill out here, but *highly* recommended you write out and reflect on your answers. Keep track of these for your own awareness and growth. At the end of the day, I can’t make these changes for you. Maintaining healthy habits and momentum ultimately is a result of holding yourself accountable and your self-regulation. Practicing a formal form of self-monitoring helps to reinforce the habits (or lack there of) so you can eventually do these tasks more automatically or do them better next time around.
